Scheduling a consultation for your patient is a simple and hassle-free process. All you need to do is complete our referral form and dispatch it to our office, accompanied by any available current radiographs. To ensure an accurate diagnosis, we kindly request that these radiographs are no more than a year old. However, should you require updated films, our office can readily accommodate your needs.
